Considering he was taking pictures of a bunch of spiders in the first movie (and I'm out of touch with the world so I've never read the comics), I would assume a a more accurate idea of his "only fear" would be failing to protect a dear friend or loved one, or having to unwillingly face a seemingly impossible and life-threatening trial for a similar reason to the other possibilty, but in any case...
